How long has the NYT had this feature? Has this omission always been the case? Surely, if it was around back then, not Leonard Bernstein ...?

The Lives They Led version of the Magazine doesn't go back as far as 1990, the year Lenny died, but I can't remember exactly when it started. Yes, surely he would have been included.

To add further insult, my newspaper's magazine section had an article about what a great loss 2016 was to music, with a list of 32 musicians who'd died during the year. The only classical name on the list was Marriner, probably because he made the record charts. The only others on the list with even the remotest connection with classical music were Sir George Martin and Greg Lake.

The Times had an "artists we lost" section and, as Alex Ross noted, ""Culture" = pop music and Hollywood (Albee, Eco, Lee had famous film adaptations). Plus Zaha Hadid."
